Miami-Dade County by the numbers
Every figure in this table comes from the CMS landscape files for the two plan years, counted the same way on both sides. Medicare Advantage and MA-PD figures exclude Special Needs Plans, which are counted on their own row and have their own table further down.

What changes in Miami-Dade County for 2027
Comparing the 2026 and 2027 landscape files for Miami-Dade County plan by plan, here is what actually moved:
- 5 fewer Medicare Advantage plans — 46 in 2026, 41 for 2027. 30 plan records from 2026 are gone from the 2027 file; 16 are new.
- $0-premium plans — 39 in 2026, 35 for 2027, counting the Part C premium on the medical-only plans that have no Part D component. A $0 premium is not the same as a cheap plan: look at the out-of-pocket maximum and the drug deductible in the same row, because that is where a $0-premium plan recovers the difference.
- Deductibles and out-of-pocket maximums both rose — the average Part D deductible on a Miami-Dade Medicare Advantage plan went from $269.86 to $381.03, and the average out-of-pocket maximum to $4,555, with the highest on the shelf at $9,850. The statutory Part D deductible ceiling rose from $615 to $700.
- Stand-alone drug plans got more expensive — Florida’s Part D region goes from 10 stand-alone plans to 11, and from 2 $0-premium plans to none. The annual out-of-pocket cap on covered drugs rises from $2,100 to $2,400.
Your Annual Notice of Change is the document that matters
If you are already in a Medicare Advantage or Part D plan, your carrier was required to send you an Annual Notice of Change (ANOC) by September 30 for coverage starting January 1. It is not marketing — it is the legal statement of what your own plan is changing. Four lines are worth finding: the monthly premium, the annual drug deductible, the in-network maximum out-of-pocket, and whether your plan is being consolidated into another one. If the ANOC says your plan is ending or merging and you do nothing, you will be moved somewhere you did not choose.
For the rules behind all of this, CMS publishes the official handbook each year: Medicare & You 2027 (PDF, medicare.gov).
Plans leaving the Miami-Dade shelf
30 plan records present in Miami-Dade County for 2026 do not appear for 2027. Some are genuine withdrawals; many are consolidations, where a carrier folds two or three plans into one and moves the members across. Until CMS publishes the plan crosswalk for 2027, the landscape file alone cannot tell you which is which for a specific plan — so we do not label any individual plan “terminated” here. What we can say is which carriers have fewer plan records on the 2027 shelf than they had on the 2026 one.
Carriers with at least one 2026 plan record not on the 2027 shelf: Aetna Medicare · Align Senior Care · CarePlus Health Plans, Inc. · Doctors HealthCare Plans, Inc. · Gold Kidney Health Plan · HealthSpring · Humana · Simply Healthcare Plans, Inc. · UnitedHealthcare · Wellcare
Carriers with plan records new for 2027: Aetna Medicare · Align Senior Care · Devoted Health · Doctors HealthCare Plans, Inc. · Gold Kidney Health Plan · Humana · Simply Healthcare Plans, Inc. · Wellcare
Where Miami-Dade County’s Medicare population actually is
Plan counts tell you what is for sale. Enrollment tells you what people here actually chose. These are CMS figures for Miami-Dade County as of June 2026.
| Group | People | Share |
|---|---|---|
| All Medicare beneficiaries in the county | 524,508 | 100% |
| In a Medicare Advantage or other health plan | 394,671 | 75.2% |
| On Original Medicare | 129,837 | 24.8% |
| With drug coverage through a Medicare Advantage plan | 391,670 | 74.7% |
| With a stand-alone Part D drug plan | 78,335 | 14.9% |
| Dual eligible (Medicare and Medicaid) | 224,591 | 42.8% |
| Receiving Part D Extra Help (low-income subsidy) | 234,316 | 44.7% |
Miami-Dade is the most Medicare Advantage–heavy of the three counties: 75.2% of beneficiaries are in a Medicare Advantage or other health plan, and 42.8% are dual eligible — on both Medicare and Medicaid. That is why 60 of the plans available here for 2027 are Special Needs Plans, more than double the number of regular Medicare Advantage plans.

Is an HMO the right shape for me?
An HMO is the tightest and usually the cheapest. You pick a primary care physician inside the network, referrals go through them, and out-of-network care is generally not covered except for emergencies and dialysis. It is a good fit if your doctors are all in one system and you do not spend months out of state. It is a poor fit if you travel, split the year between two homes, or see specialists across several systems.

Is an HMO-POS the right shape for me?
An HMO-POS is an HMO with a point-of-service option — a defined list of services you can get out of network, at a higher cost share. It is a middle ground, but the list is specific and usually short, so read what is actually included rather than assuming it means general out-of-network coverage.

Is a PPO the right shape for me?
A PPO lets you see out-of-network providers without a referral, at a higher cost share, and does not require a primary care gatekeeper. That flexibility normally shows up as a higher premium or a higher out-of-pocket maximum. It is the usual answer for people who travel, winter here, or want to keep a specialist outside the plan’s main system.

Is a Regional PPO the right shape for me?
A Regional PPO covers a whole CMS region rather than a single county, with one uniform benefit package and premium across it. If you move between Florida counties through the year, that uniformity is the point. Networks are generally broad but the out-of-pocket maximums here run at the high end of the shelf.
Special Needs Plans (60 for 2027)
Special Needs Plans are restricted to people who meet a specific condition: being on both Medicare and Medicaid (D-SNP), having a qualifying chronic condition (C-SNP), or living in an institution (I-SNP). They are listed separately because their premiums, deductibles and out-of-pocket maximums are not comparable with regular Medicare Advantage plans, and averaging them together distorts both.

Premiums are the monthly figure the plan charges, from the CMS 2027 landscape file: the consolidated Part C + D premium where the plan includes drug coverage, and the Part C premium where it does not. Maximum out-of-pocket is the in-network annual limit. Star ratings are the contract’s 2026 overall rating. Contract-Plan IDs let you look any row up on medicare.gov.
How do I know whether I qualify for one of these?
D-SNP eligibility follows your Medicaid status, including partial Medicaid through a Medicare Savings Program — and 224,591 people in Miami-Dade County, 42.8% of all beneficiaries, are dual eligible. C-SNP eligibility needs your doctor to confirm the qualifying condition. I-SNPs require institutional residence. If you think you may qualify but have never been in one, that is worth checking: eligibility can change without anyone telling you, and a D-SNP will usually beat a regular plan on cost if you qualify.
Plan ratings in Miami-Dade County
CMS rates each Medicare contract from 1 to 5 stars on clinical quality, member experience and customer service. Ratings apply to the contract, not the individual plan, so every plan under one contract carries the same overall rating. The distribution below covers the Medicare Advantage plans available in Miami-Dade County and excludes Special Needs Plans.
| Overall rating | Plans | Share |
|---|---|---|
| 5★ | 6 | 13.0% |
| 4.5★ | 19 | 41.3% |
| 4★ | 7 | 15.2% |
| 3.5★ | 14 | 30.4% |
The average is 4.18 stars across 46 rated plans, and 69.6% are at 4 stars or better. 0 plans have no rating because their contract is too new to have one — that is not a bad score, it is the absence of a score, and it is worth treating differently from a low one.

Which systems the plans build around
Miami-Dade has more hospital systems competing than either neighboring county, and plan networks differ sharply between them. Which system your physicians belong to is usually the first question worth answering.
- Jackson Health System — Jackson Memorial, Jackson North, Jackson South, Jackson West. The public system and the safety net; central to most Special Needs Plan networks.
- Baptist Health South Florida — Baptist Hospital, South Miami, Doctors Hospital, West Kendall Baptist, Homestead, Mariners. The largest private system in the county and a common dividing line between plans.
- University of Miami Health System — UHealth Tower and Sylvester Comprehensive Cancer Center. If you are in cancer treatment at Sylvester, check it by name: participation is narrower than general hospital coverage.
- Mount Sinai Medical Center (Miami Beach) and HCA Florida Kendall and Mercy — all widely but not universally in network.
- Several carriers here run their own clinic networks — Leon Health, HealthSun, CarePlus, Solis, Freedom and Optimum among them. Those plans can be excellent value if you are willing to use their clinics, and a poor fit if you are not.

A filter narrows the shelf; it does not recommend a plan. Whether a plan works for you depends on your prescriptions, your doctors and your budget, and the only way to settle that is to check your own drug list and provider list against the specific plan.
How and when to enroll
Four windows cover almost every situation. Enrolling at the wrong time is the most common avoidable mistake in Medicare, and some of it cannot be undone until the following year.
| Window | Dates | Who it is for |
|---|---|---|
| Annual Enrollment Period | October 15 – December 7, 2026 | Anyone on Medicare. Change Medicare Advantage or Part D plans for coverage starting January 1, 2027. The last choice you make by December 7 is the one that takes effect. |
| Medicare Advantage Open Enrollment | January 1 – March 31, 2027 | People already in a Medicare Advantage plan. One switch to another Medicare Advantage plan, or back to Original Medicare with a stand-alone Part D plan. Not available if you are on Original Medicare. |
| Non-renewal Special Enrollment Period | December 8, 2026 – end of February 2027 | Members whose plan is not being renewed for 2027. Extra time beyond the Annual Enrollment Period. Your Annual Notice of Change is what tells you whether this applies to you. |
| Initial Enrollment Period | The 7 months around your 65th birthday | People new to Medicare — three months before the month you turn 65, that month, and three months after. Late Part B and Part D enrollment carries a penalty that lasts as long as you have the coverage. |

Questions about Medicare in Miami-Dade County
How many Medicare Advantage plans are available in Miami-Dade County for 2027?
41 Medicare Advantage plans, down from 46 for 2026, plus 60 Special Needs Plans for people who are dual eligible, have a qualifying chronic condition, or live in an institution. 35 of the Medicare Advantage plans carry a $0 monthly premium.
Is a $0-premium Medicare Advantage plan in Miami-Dade County really free?
No. You still pay your Medicare Part B premium to Medicare, and a $0-premium plan still has copays, coinsurance and an out-of-pocket maximum. In Miami-Dade County for 2027, the average in-network out-of-pocket maximum is $4,555 and the highest on the shelf is $9,850. A $0 premium tells you what you pay in a month when nothing happens; the out-of-pocket maximum tells you what a bad year costs.
My Miami-Dade County plan is being discontinued. What happens if I do nothing?
If your plan is ending outright, you go back to Original Medicare on January 1 with no drug coverage, which can trigger a Part D late enrollment penalty. If your plan is being consolidated into another plan, you are moved into the successor plan automatically — which may have a different network, a different drug list and a different premium. Either way you also get a Special Enrollment Period from December 8 through the end of February to choose something yourself.
Do the star ratings on this page apply to 2027?
No, and we have labeled them accordingly. CMS releases new ratings in October and the 2027 landscape file ships with the rating column empty, so every star figure here is the contract’s 2026 overall rating. Across Miami-Dade County’s Medicare Advantage plans, 69.6% are at 4 stars or better on that basis. We update the page when the 2027 ratings publish.
Why are there so many Special Needs Plans in Miami-Dade?
Because 42.8% of the county’s 524,508 Medicare beneficiaries are also on Medicaid — 224,591 people. Dual Eligible Special Needs Plans (D-SNPs) are built for exactly that group, and carriers compete hard for it here. If you have both Medicare and Medicaid, or you qualify for a Medicare Savings Program, the D-SNP shelf is usually where the best value sits, and it is worth reviewing even if you have been in the same plan for years.

star_ratings_2026.zip. Published 2025-10-09. Accessed 2026-10-12. The newest star ratings CMS has published. CY2027 ratings are blank in the landscape file until the October 2026 release, so every star figure on this page is the 2026 rating of the contract that carries the plan into 2027. -

How these numbers were produced
- Plan counts, premiums, deductibles and out-of-pocket maximums are counted directly from the CMS landscape source files for Miami-Dade County, one row per contract-plan-segment, for both plan years. Nothing is modeled or estimated.
- Every Medicare Advantage and MA-PD aggregate excludes Special Needs Plans. SNPs are counted on their own row and in their own table. Mixing them moves the premium and out-of-pocket averages a long way, because SNP cost sharing is built around Medicaid wrap-around rather than a retail premium.
- Average Part C premium and average consolidated Part C + D premium are reported as two separate rows and never combined.
- Star ratings are the contract’s 2026 overall rating, the newest CMS has published. The 2027 landscape file leaves the rating column empty until the October Star Ratings release. Plans whose contract has no rating are shown as unrated rather than being dropped from the count.
- Enrollment figures are county totals from the CMS Medicare Monthly Enrollment file for June 2026. CMS does not publish plan-level enrollment by county in the files behind this page, so this page reports no “most popular plan” ranking rather than estimating one.
- Plans are described as leaving or arriving on the shelf purely on whether a matching contract-plan-segment record exists in each year’s file. Because the 2027 CMS plan crosswalk has not published, no individual plan is labeled terminated and no successor plan is named.
- This page is a reading of public CMS data, not plan advice. Benefits, networks and formularies are set by the carriers and change; the plan’s own Evidence of Coverage governs.
