What is a D-SNP and who qualifies?

A D-SNP, or Dual Eligible Special Needs Plan, is a Medicare Advantage plan that only people with both Medicare and help from Medicaid can join. You qualify if you have Part A and Part B, live in the plan’s service area, and are in a Medicaid group the plan accepts.

Last reviewed

Help from Medicaid is broader than full Medicaid. CMS lists full Medicaid and each of the Medicare Savings Programs, which help pay Medicare costs: QMB, SLMB, QI and QDWI, with or without other Medicaid. A plan can be limited to some of those groups, so one D-SNP may take only people with full Medicaid while another also takes people with a Medicare Savings Program alone. Every D-SNP includes Part D drug coverage, and you can stay in one only while you keep meeting its eligibility rules.

How a D-SNP works

A D-SNP covers everything Original Medicare covers, adds Part D, and can help coordinate your Medicare and Medicaid benefits. 28 of Broward’s 33 are HMOs or HMO-POS plans, which means a provider network. CMS asks every D-SNP to declare whether it is a Medicare zero-dollar cost sharing plan, and 16 of the 33 in Broward are. The need is large here: in June 2026, 83,971 people with Medicare in Broward were dually eligible, 37,640 with full Medicaid and 46,331 with partial help.

What changes in 2027

From 2027 a federal rule ties some D-SNPs to Medicaid managed care. Where the company behind a D-SNP, or a company with the same parent, also runs a Medicaid managed care plan for people with full Medicaid in the same area, it may offer only one D-SNP for them there, and new members must be enrolled in, or enrolling in, that company’s Medicaid plan. From 2030 those D-SNPs may cover only people in the matching Medicaid plan. If your full Medicaid comes through a managed care plan, the company that runs it now matters when you choose a D-SNP.

What to do next

Start with your Medicaid notice, because the group it puts you in decides which D-SNPs will take you. Then check which company runs your Medicaid plan, since from 2027 that can narrow your choice, and compare the D-SNPs open to you on the doctors in their networks and the drugs on their 2027 lists. Keep your Medicaid renewal current: a D-SNP can keep you only while you meet its rules, and if you lose eligibility you may get a Special Enrollment Period to change plans.

Related questions

Can I join a D-SNP with only a Medicare Savings Program?

You may be able to. CMS counts QMB, SLMB, QI and QDWI among the Medicaid groups a D-SNP can serve, but each plan can be limited to some of them, so check what the plan accepts.

Do D-SNPs include drug coverage?

Yes. Every Special Needs Plan, including every D-SNP, must include Medicare Part D drug coverage.

Do I pay a premium for a D-SNP in Broward?

For most of them, no. 29 of the 33 D-SNPs offered in Broward for 2027 have a $0 monthly premium. The Part B premium is separate, and Medicaid or a Medicare Savings Program may pay it for you.
